Same-day and next-day times are often available. Simply request one when you call.

 

Extended and Weekend Hours

Many of our Family Practice, Pediatrics, and Internal Medicine providers offer night and weekend hours. Call (920) 496-4700 for specific details.

  

Prevea On-Call

Our phones are answered 24-hours a day, seven days a week. If you're not sure whether you need to see a doctor right away, the Prevea Health on-call operator will direct you to someone who can help.

Urgent Care

Just walk right in. At Prevea Urgent Care, no appointment or referral is necessary. You do not need to be a PRevea patient. All ages, including infants, are welcome. And at Prevea Urgent Care, a board certified physician is always on-site to ensure the highest quality of care. Learn More >

FirstCare

FirstCare provides 24-hour-care for patients experiencing a non-emergency injury or illness. No appointment is necessary. Simply walk in. FirstCare is located in the emergency departments at St. Mary's Hospital Medical Center and St. Vincent Hospital.

 

 

                   

      

    

Insurance Information

New Patients, New Coverage, or Changes in Coverage
Before your first appointment, we will ask you to complete an insurance form and provide proof of insurance. During future appointments, we will confirm that your insurance coverage remains unchanged.

 

If you know that your coverage changed, please allow time before the appointment to update your records. We submit claims to insurance carriers daily. Please alert us of any changes prior to your appointment to avoid incorrect billing.

 

We file your insurance claims as a courtesy. Ultimately, you will need to pay for items, tests and services not covered by insurance. Please contact your insurance carrier directly to resolve coverage issues.

 

Benefit or Claim Questions
If you have a benefit, coverage or claim question, contact your employer's benefits office, health insurance carrier or claims administrator.
