Medical Insurance
Offered to employees classified with either full- or part-time status. Coverage is available at competitive rates to employees, their spouses and/or their children.
 
Health Savings Account (HSA)
Provides an opportunity for a qualified High Deductible Health Plan (HDHP) participant to obtain a tax benefit on savings earmarked for medical uses.
 
Dental Insurance
Offered to employees classified with either full- or part-time status, for individual purchase. Three plan options are offered, and coverage is available to employees, their spouses and/or their children.
 
Life Insurance/AD&D
Offered to employees classified with either full- or part-time status, with costs paid by Prevea Health.
 
Optional Life Insurance/AD&D
Offered to employees classified with either full- or part-time status, for individual purchase. Coverage is available to employees, their spouses and/or their children.
 
Short Term Disability
Offered to employees classified with full-time status, with costs paid by Prevea Health.
 
Long Term Disability
Offered to employees classified with full-time status, with costs paid by Prevea Health.

Flexible Spending (Medical, Limited Medical and/or Dependent Care)
Provides an opportunity for employees classified with either full- or part-time status to save pre-tax dollars for medical, limited medical and/or dependent care costs.
 
401(k) & Retirement Plan
Offered to employees, age 18 or older who are classified with either full- or part-time status and who have successfully completed eligibility requirements. The plan offers an efficient means of enhancing retirement income.  
  • Employee Contributions: Employees can begin deferring contributions to the plan on the first of the month following a one-month waiting period.
  • Employer Contributions: Employees may be eligible for an Employer Match and a Discretionary Employer Contribution after completion of the eligibility requirements.

Employee Assistance Program (EAP)
Offered to all Prevea employees and their immediate family members (those residing in the employee’s primary residence). The program provides assistance with personal, family and/or professional issues.
 
Paid Time Off (PTO)
Offered to employees classified with either full- or part-time status, on an accrual basis, to provide for paid time away from work, when approved by a supervisor. Time for vacations, holidays, personal days, weather emergencies, family emergencies and illness/injury are combined together within one bank of PTO hours.
 
Tuition Reimbursement
Offered to employees classified with either full- or part-time status. Tuition reimbursement covers a portion of the expense for classes in degree programs related to the individual’s profession at Prevea Health.
