The medical benefits portion of the medical plans are administered by Dean Health Plan Administrative Services (aka Dean Care/ASO) and the pharmacy benefits portion of the medical plan is administered by CVS Caremark. 

Prevea’s medical plan provides for quality health care and 100% coverage for in-network preventive care. Premiums are based on the number of people you cover, and your employment status (full-time, part-time). All eligible employees have access to our Traditional EPO or EPO high deductible health plan (HDHP) administered through Dean Health Plan Administrative Services. Those benefit eligible employees whom live outside of the Prevea360 provider network have access to enroll in our PPO high deductible health plan (PPO HDHP). The PPO HDHP medical plan uses the First Health Network.

After you have enrolled for medical plan coverage, watch your mail as you will receive an ID card. You will need to present this card to obtain medical and prescription drug services. Your ID card acts as both your medical and prescription ID Card.

Traditional EPO & EPO HDHP Medical Plan – Preferred Provider Network

For the Prevea Traditional EPO & EPO HDHP Medical Plans (these medical plans are available to employees who reside in the Prevea360 service area), the Prevea medical plan uses the Prevea360/ASO provider network.

How to find an In-Network Prevea360 Provider:

  4. Under Select Plan Type click ASO Network

PPO Medical Plan – Preferred Provider Network

For the Prevea PPO Medical Plan (the medical plan made available to employees who reside in Wisconsin, but outside the Prevea360 service area), the medical plan uses the First Health Network. For employees/families who reside outside of Wisconsin, the medical plan uses the First Health Network network of providers.

How to find an In-Network First Health Provider:

  4. Click Locate a Provider or Create a Directory
  5. Under Network Options click First Health Network
  6. Select Provider Type

Special note regarding Out-of-Area Dependents and Prevea Health EPO Medical Plan Coverage (Traditional EPO & EPO HDHP)

We offer coverage for your eligible dependents even if they reside outside the Prevea360 ASO network/service area. The employee or out-of-area dependent must contact Dean Health prior to receiving any services as follows:
  1. Call the Dean Customer Care Center at (877) 234-4516 (TTY: 711) Monday through Friday, 7:30 a.m. to 5 p.m.
  2. Explain that your dependent is residing out-of-area and provide the address and phone number of the out-of-area dependent
  3. The Customer Care Center will update your dependent’s information to indicate that they reside outside of the service area

Prescription drug coverage through National Rx/Caremark

For the Prevea EPO and PPO Medical Plans, as mentioned earlier, pharmacy benefits are administered through National Rx/Caremark. To obtain reimbursement under the plan for pharmacy related expenses it is important to ensure your using an in-network pharmacy.

How to find an in-network pharmacy

CVS Caremark makes it easy to fill your prescriptions with retail network pharmacies around the United States. Follow these online instructions to locate an in-network pharmacy in your area:
  1. Visit to get started
  2. Register and create an account (our member ID as found on your ID card or SSN will be needed)
  3. After registering, click Pharmacy Locator in the upper right-hand corner of your dashboard
  4. Enter your ZIP code or city/state to find an in-network pharmacy near you

If you have any specific questions regarding participating pharmacies or other questions related to covered medications, call CVS Caremark Customer Service at (866) 818-6911 (open 24 hours a day, 7 days a week).

To ensure plan members are spending health care dollars wisely, CVS can help:

  • Confirm coverage

Once registered for the website as mentioned above, enter your drug name, strength and quantity needed to determine if the medication is covered under the plan, and if so, information on the member cost will be shown. If a lower cost covered medication could be an alternative for the original medication that was searched, that information will also be displayed at the bottom of the search results.

Preventative services
Regular checkups can improve your health and extend your life. By getting the recommended exams and screenings, you increase your chances of discovering problems before an illness significantly affects your health. Preventive care is beneficial not only to your physical well-being, it also makes sense for your financial health because generally it’s covered at 100% by Prevea’s medical plan when rendered by in-network providers. For more information on preventive care benefits please refer to your Prevea Health Medical Summary Plan Description (SPD). 

In the EPO HDHP and PPO HDHP, click here for a list of prescriptions listed on our Preventative Drug List. The medications included on the Expanded Preventive Drug List are paid at 100% by the plan and are no cost to you. This does NOT apply for the Traditional EPO

Member Portal Access: Register at to get started!

Your Dean Care Member Portal provides a complete dashboard view of your medical and pharmacy plan and self-service tools. The Member Portal keeps your health care information secure and strictly confidential. This site navigation and helpful features go hand in hand with the exceptional customer service you’ve come to expect from Dean Health Plan.

  • Print a temporary ID card
  • Search for an in-network doctor or facility
  • View claims status and history information
  • Download important forms and documents
  • View deductible balances

How to register:

  • View to get started
  • Click Member Portal
  • Click Register Now; read the License Agreement and click Agree
  • Enter your date of birth, ZIP code and Member ID (xxxxxxxxx-xx)
  • Create a user password (must be at least 8 alphanumeric characters), then enter a secure question and answer

Virtual Care

Treatment for minor conditions is available with no appointments necessary. Virtual Care is a convenient and affordable option for a variety of medical services. Access quality health care online from the comfort of home, during your lunch break or while traveling.

General medicine
  • $15* per visit will apply for Prevea Health employees, their spouses and dependents covered under the Prevea-sponsored medical plan
  • Board-certified doctors are available by web
  • Treat flu allergies, sinus infection, rash, sore throat and more

How it works
  • Visit to complete an online health interview 
  • A provider reviews the interview usually within one hour and creates a treatment plan
  • You are notified when the results are available

Special features
  • Accessible 24/7
  • If you cannot be treated online, you will not be responsible for the $15 fee
  • If a prescription is part of your treatment plan, you’ll be able to send your prescription to the pharmacy of your choice

*The $15 fee does not apply towards your medical deductible and max out-of-pocket amounts.
