Prevea COVID-19 Vaccination Clinic - Rice Lake

Prevea Rice Lake Health Center will launch a community COVID-19 vaccination clinic starting March 17, 2021. All appointments are required and must be scheduled through MyPrevea. Scheduling is based upon vaccine allocation received from the State of Wisconsin.

Take WI-25 N. Turn right onto 13 1/2 Ave/E Division Ave. Turn left onto 6th St. Continue onto WI-25 N/N 7th St. Turn right onto WI-48 E/21st Ave/Knapp St. At the traffic circle, take the 2nd exit onto 19th St/West Ave. Your destination will be on the right.

From 48 West
Take WI-48 E/22 1/2 Ave. At the traffic circle, take the 2nd exit onto 19th St/West Ave. Your destination will be on the right.

From 53 North
