Class 1: Heart Failure 101
An experienced team of cardiology providers and registered nurse care managers will help you gain a better understanding of heart failure, including what symptoms you should watch for and what to do about them.
Class 2: Medication Education
You may be taking multiple medications and for many different reasons, but do you know why? Registered pharmacists from St. Vincent and St. Mary’s Hospitals will talk about common medications, how to take them, what side effects they have and what to watch for. If you’d like, a pharmacist will meet with you one-on-one to answer questions about your specific medications. Please bring them along in their original bottles.
Class 3: Fitness for Your Heart
Being active and functional is very important for your heart, body and mind. Learn what activities are good and not so good for you, and more importantly: how to stay active. Experienced exercise physiologists have an in-depth understanding of how your heart and entire body works, and will be able to tailor an activity plan just for you.
Class 4: Nutrition – Fueling Your Heart
Just like your car, your body needs fuel (food) to function, and the food choices you make can significantly affect how you feel. Prevea’s registered dietitians will discuss portion control, how salt affects your body, good food choices, how to read food labels and making smart decisions at the grocery store.
Class 5: Check-up from the Neck Up
A behavioral care therapist from Prevea Health and a registered nurse and social worker from Unity will show you how to make your day-to-day living better. Find out the beneficial ways Prevea and Unity can help you cope with physical and mental lifestyle changes, reduce the amount of stress on you and your family, and why it’s important to rely on inner strength and humor.
