We partner with four hospitals: HSHS St. Vincent Hospital and HSHS St. Mary's Hospital Medical Center in Green Bay; HSHS St. Nicholas Hospital in Sheboygan; and, HSHS St. Clare Memorial Hospital in Oconto Falls. We share one combined electronic medical record with our hospital partners, so wherever a patient is seen--in one of our health centers or hospitals--his or her complete medical record will be readily available to all treating providers. This ensures the highest continuity of integrated care.
