An Update on Prevea’s Western Wisconsin Operation

With MyPrevea—a convenient, secure and confidential online resource—you have access to your personal health information from any computer or mobile device at any time.

With proper consent, you may use features of MyPrevea to access the health information of a child. If you do not have your own MyPrevea account, you will need to create one in order to request proxy access for another patient’s health information.

CHILDREN UP TO AGE 12

  • Refill and track prescriptions
  • View their immunization records
  • View their list of allergies
  • View lab and test results as soon as they're available
  • Schedule an appointment
  • Message their provider

CHILDREN AGE 13 – 17*

Limited access is available for children age 13 to 17.

  • Schedule an appointment
  • Message their provider
*Due to Health Insurance Portability and Accountability Act (HIPAA) regulations, we are not able to offer full proxy MyPrevea access for children ages 12 to 17. Limited proxy access allows the ability to schedule appointments, change the address or other account information, request medical advice or enter your child’s information in the “Track My Health” feature of MyPrevea.

How to request proxy access for a minor:

  1. Log in to your MyPrevea account
  2. Click “Menu”
  3. Under “Account Settings”, click “Proxy Access”
  4. Complete the “Request to Access a Minor’s Record” form and click “Submit Request”

MyPrevea is an extension of your medical record, and as such, your privacy is strictly protected. Your name, address, email, phone number and all other personal information is confidential and will never be sold or leased.
