Prevea Western Wisconsin health centers have closed effective Friday, April 19, 2024 with the exception of the Prevea Residency Clinic located at 617 W. Clairemont Ave. in Eau Claire and 207 W. Lincoln St. in Augusta as well as the Prevea location at UW-Stout which last day of patient care will be on or before June 30, 2024.

Community members should seek care at neighboring facilities moving forward. In case of emergency, call 911.

Your medical records will continue to be accessible and care you received through Prevea Health will be captured in your medical records available on MyPrevea. Prevea Health will help transition your medical records to a new facility or provider upon your request and completion of an authorization form. You may access a copy of the authorization form here.
