Prevea Western Wisconsin Operation Update

It is with a heavy heart that we announce that Prevea’s Western Wisconsin health centers, in addition to HSHS Sacred Heart Hospital in Eau Claire, and HSHS St. Joseph’s Hospital in Chippewa Falls, are closing. For more information regarding HSHS Sacred Heart and St. Joseph’s Hospitals, click here. We expect the majority of Prevea Health centers in Western Wisconsin will close on or before April 21, 2024. 

Like many health systems, we have had to grapple with the lingering impacts of the pandemic, the effects of inflation, workforce challenges and other industry-wide trends. 

As a result of these persistent operational challenges and after thorough consideration, we have made the difficult yet necessary decision to exit the Western Wisconsin region and focus our efforts on our Eastern Wisconsin operations.

We understand this news is concerning, disappointing, and even distressing to some of you. Our team is committed to providing you with information and resources to support the transition of your care. 

We want to assure you that we remain dedicated to ensuring you have access to timely, safe, and high-quality care in your community.  

 If you have questions about your care or upcoming procedures and appointments, as well as any concerns regarding your patient care plan, we encourage you to reach out to (833) 344-4373 for Prevea and (888) 307-7099 for HSHS. In case of an emergency, call 911.

The HSHS and Prevea Health teams will work with your provider to safely transition your care to another provider of your choosing prior to the closure date. You may also contact your insurance company or refer to the Doctor Finder of the American Medical Association for assistance in obtaining another physician. The Wisconsin Department of Health Services also provides assistance in identifying physicians at https://www.dhs.wisconsin.gov/guide/doctor.htm.  

Your upcoming routine appointments may be rescheduled or cancelled; please contact your provider’s office to discuss the transition of your care. If you have an emergency, you should contact 911. We are committed to managing a thoughtful and organized wind-down of these operations in Western Wisconsin, and to supporting our patients and colleagues during these changes. Your medical records will continue to be accessible and care you received through Prevea Health will be captured in your medical records available on MyPrevea. Prevea Health will help transition your medical records to a new facility or provider upon your request and completion of an authorization form. Your completed form can be dropped off at any Prevea location or mailed to: Prevea Health Services, Attn: Records Release, P.O. Box 19070, Green Bay, WI 54307. Questions about your medical records can be directed to (920) 496-4783

If you have questions about your prescriptions prior to the closure date, or any concerns regarding your overall plan of care, we encourage you to reach out to your Prevea Health or HSHS provider. For patients of Prevea Health locations expected to close on or before April 21, 2024, Prevea Health providers will ensure continuity of necessary prescription medications for a thirty (30) day period after the date of this letter, to permit you sufficient time to establish care with a new provider. After that time, prescription requests should be directed to your new provider. Prevea Health will also provide assistance in transitioning your prescription history to your new provider. 
As always, if you experience an emergency, please call 911 or utilize your local emergency department. If you experience an urgent medical need for which you would normally call a Prevea Health provider, patients of Prevea Health locations closing on or before April 21, 2024, may continue to call their providers for thirty (30) days following the date of this letter, to permit you sufficient time to establish care with a new provider. After that time, calls should be directed to your new provider.  
We want to assure you that this decision does not impact you or any other HSHS and Prevea patients in Eastern Wisconsin.

Most procedures scheduled by a Prevea Health provider at HSHS Sacred Heart Hospital after January 22, 2024 will be cancelled as our capacity will be limited in preparation for the anticipated closures. If you have a procedure scheduled, your Prevea Health provider will contact you regarding next steps in your care.
