Dean Health Plan

The medical benefits portion of the medical plans are administered by Dean Health Plan Administrative Services (aka Dean Care/ASO) and the pharmacy benefits portion of the medical plan is administered by CVS Caremark. 

Prevea’s medical plan provides for quality health care and 100% coverage for in-network preventive care. Premiums are based on the number of people you cover, and your employment status (full-time, part-time). All eligible employees have access to our Traditional EPO or EPO high deductible health plan (HDHP) administered through Dean Health Plan Administrative Services. Those benefit eligible employees whom live outside of the Prevea360 provider network have access to enroll in our PPO high deductible health plan (PPO HDHP). The PPO HDHP medical plan uses the First Health Network.

After you have enrolled for medical plan coverage, watch your mail as you will receive an ID card. You will need to present this card to obtain medical and prescription drug services. Your ID card acts as both your medical and prescription ID Card.

Network information (Traditional EPO, EPO HDHP & PPO HDHP option)

PPO Medical Plan – Preferred Provider Network 

For the Prevea PPO Medical Plan (the medical plan made available to employees who reside in Wisconsin, but outside the Prevea360 service area), the medical plan uses the First Health Network. For employees/families who reside outside of Wisconsin, the medical plan uses the First Health Network network of providers. 

How to find an In-Network First Health Provider: 

  1. Visit www.deancare.com/aso to get started 

  2. Scroll down and click See ASO for Members 

  3. Scroll down and under Find a Doctor click the link for First Health 

  4. Click Locate a Provider or Create a Directory 

  5. Under Network Options click First Health Network 

  6. Click Start Now 

  7. Select Provider Type 

  8. Enter your search criteria 

  9. Click Search Now

Prevea Health medical plans available to employees

Below is a brief outline of the medical plan designs offered to eligible employees.  Please refer to the summary plan descriptions on myADP for complete plan details. 

2023 Plan Year

During the 2023 plan year all employees enrolled in a Prevea medical plan are required to fill out the 2023 Spouse Medical Insurance Coverage Statement. The form will need to be completed, signed by the spouse’s employer, and returned to Prevea Human Resources- Benefits, within 30 days of enrollment for plan year 2023, in order to avoid a spousal surcharge.

Prevea Partnered Health (PPH) discounts are available to those enrolled in the EPO HDHP and Traditional EPO. Appointments in urgent care, primary care (family medicine, internal medicine, and pediatrics) and physical and occupational therapy are available at these locations.

Do you have a primary care physician?

Establishing a relationship with a primary care physician (PCP) is important. Your PCP will be familiar with your medical history, coordinate all aspects of your care.  A PCP includes those who practice internal medicine, are family or general practitioners, or are OB-GYNs.  Together, you and your physician determine how best to improve your overall well-being.  Please note that Prevea Clinic will not have access to any specific, personally identifiable health information. 

Program Instructions:

  • Find an In-Network Prevea360 Provider online at www.deancare.com/aso if you don’t already have one. Scroll down and click  See ASO for Members.  From there, scroll down and under Find a Doctor, click the link for Prevea360 ASO Network.  Under Select Plan Type click ASO Network.  Enter your provider search criteria.  You can also call Dean Care Customer Service at 877-234-4516 for assistance.
  • Call to schedule a routine physical exam visit.  Tell your doctor’s office that the visit should be coded as a routine physical exam.  Your health plan through Prevea Clinic will pay for one preventative/routine physical exam visit per year at 100%.
  • Complete the routine physician exam with your PCP between 11/01/2022 to 10/31/2023
  • At your visit, have your physician complete and sign the Prevea PCP Engagement form.
  • If you have already had a routine physical exam between 06/01/2022 to 10/31/2022, this visit will qualify for meeting the criteria for plan year 2024 premium incentive.  Please see instructions on the Prevea PCP Engagement form.
  • Submit completed forms by no later than 11/01/2023
    • Return your form to Human Resources- Benefits Team either via interoffice mail to HR-Benefits Team @ Hansen/HR or Fax to: 920.496.4717 Attn: HR_ Benefits Team.

A monthly employee medical plan premium surcharge will apply effective 01/01/2024 if you do not participate in the 2023 PCP engagement program.   

Covered employees, hired on or after Nov. 2, 2022, are not required to participate in the program in 2023, however will be subject to a medical plan premium surcharge if not participating in future plan years (2024, etc.). 

In order to show proof of participation in the PCP engagement program, you will need to take the Prevea PCP Engagement form to your preventive doctor’s visit and submit to HR-benefits as proof of participation in the 2023 program. If no form is submitted a non-PCP engagement surcharge will apply to the employee for plan year 2024. Click Here for the Prevea PCP Engagement form.

2024 Plan Year

For the 2024 plan year all employees who have a spouse enrolled in a Prevea medical plan are required to fill out the 2024 Spouse Medical Insurance Coverage Statement. The form will need to be completed, signed by the spouse’s employer, and returned to Prevea Human Resources- Benefits, within 30 days of enrollment for plan year 2024, in order to avoid a spousal surcharge.

Call to schedule a routine physical exam visit or have a follow up on a chronic condition with a PCP (Family Practice, Internal Medicine, or OB/GYN).  

If you are enrolled in Prevea’s Medical Plan please complete the routine physical exam or follow up on a chronic condition with your PCP between 11/01/2023 to 10/31/2024 to avoid a surcharge for the 2025 plan year. 

At your visit, have your provider complete and sign the 2025 Prevea PCP Engagement

Submit completed forms by no later than 11/01/2024. 

Return your form to Human Resources- Benefits Team either via interoffice mail to HR-Benefits Team @ Hansen/HR or Fax to: 920.496.4717 Attn: HR_ Benefits Team. 

A monthly medical plan premium surcharge ($50.00 per month) will apply effective 01/01/2025 if you do not participate in the 2025 PCP engagement program.    

If you are hired in the 2024 plan year you are not required to participate in the PCP Engagement Program for the 2025 plan year, however will be subject to a medical plan premium surcharge if not participating in future plan years (2026, etc.).

Prescription drug coverage through National Rx/Caremark

To ensure plan members are spending health care dollars wisely, CVS can help:

  • Search for a drug
  • Confirm coverage
  • Understand cost
  • Explore lower-cost options
Once registered for the www.caremark.com website as mentioned above, enter your drug name, strength and quantity needed to determine if the medication is covered under the plan, and if so, information on the member cost will be shown. If a lower cost covered medication could be an alternative for the original medication that was searched, that information will also be displayed at the bottom of the search results.

CVS Caremark makes it easy to fill your prescriptions with retail network pharmacies around the United States. Follow these online instructions to locate an in-network pharmacy in your area:

  1. Register and create an account (our member ID as found on your ID card or SSN will be needed)
  2. After registering, click Pharmacy Locator in the upper right-hand corner of your dashboard
  3. Enter your ZIP code or city/state to find an in-network pharmacy near you

If you have any specific questions regarding participating pharmacies or other questions related to covered medications, call CVS Caremark Customer Service at (866) 818-6911  (open 24 hours a day, 7 days a week).
For the Prevea EPO and PPO Medical Plans, as mentioned earlier, pharmacy benefits are administered through National Rx/Caremark. To obtain reimbursement under the plan for pharmacy related expenses it is important to ensure you're using an in-network pharmacy.

Virtual Care

Treatment for minor conditions is available with no appointments necessary. Virtual Care is a convenient and affordable option for a variety of medical services. Access quality health care online from the comfort of home, during your lunch break or while traveling.
  • $15* per visit will apply for Prevea Health employees, their spouses and dependents covered under the Prevea—sponsored medical plan
  • Board-certified doctors are available by web
  • Treat flu allergies, sinus infection, rash, sore throat and more

*The $15 fee does not apply towards your medical deductible and max out-of-pocket amounts.

  • Visit www.prevea.com/virtualcare to complete an online health interview 
  • A provider reviews the interview usually within one hour and creates a treatment plan
  • You are notified when the results are available
  • Accessible 24/7
  • If you cannot be treated online, you will not be responsible for the $15 fee
  • If a prescription is part of your treatment plan, you’ll be able to send your prescription to the pharmacy of your choice
