Manitowoc and Oostburg – Prevea Health is pleased to welcome Sarah Weiss, MD, and Anastasia Mathis, APNP, FNP-C, now caring for patients at select health centers in the Lakeshore community.
Dr. Weiss sees patients at both the Prevea Oostburg Health Center, located at 15 South 10th St., Suite A in Oostburg and the Prevea Manitowoc Health Center, located at 4810 Expo Drive in Manitowoc. As a family medicine physician, she provides routine medical care for children and adults; yearly physicals; well-child visits; preventive medicine; medication management; women’s care and minor office procedures.
“My favorite part of family medicine is getting to build relationships with patients during their lifetime,” said Dr. Weiss. “My hope is patients leave each appointment feeling like they were heard, and they are comfortable returning for further health care when needed.”
Joining Dr. Weiss at the Prevea Manitowoc Health Center is family medicine nurse practitioner, Anastasia Mathis. She provides routine medical care for children, adults and elderly; preventive medicine; well-child exams; physical exams and health screenings; sports physicals; medication management; management of acute illness and chronic disease management.
“My goal is to develop a strong patient-provider relationship to meet each patient’s holistic health goals,” said Mathis. “I strive to thoroughly educate each patient so they can confidently make informed health choices and optimize their quality of life.”
