Sheboygan – Prevea Health announces the following changes for its COVID-19 testing services in Sheboygan Co.

Starting Monday, April 12, COVID-19 testing with Prevea Health in Sheboygan Co. will be provided at the following locations:

  • Prevea Sheboygan Health Center, 1411 N. Taylor Dr., Sheboygan
  • Prevea Plymouth Health Center, 825 Walton Dr., Plymouth

The last day of testing at the Prevea COVID-19 testing site at the Southtown Mall, 3335 S. Business Dr. in Sheboygan will be Sunday, April 11.

COVID-19 testing will also continue to be available at Prevea locations in the Green Bay area, Kewaunee, Marinette, Oconto Falls, Shawano, Sheboygan and in Western Wisconsin’s Chippewa Valley. A complete list of Prevea Health COVID-19 testing locations can be found at:  www.prevea.com/COVID-19

COVID-19 testing through Prevea Health is provided by appointment only and at no charge, in partnership with the Wisconsin Department of Health Services. You do not have be an established patient at Prevea Health to receive COVID19 testing through Prevea Health. How to make an appointment:

  1. Create a MyPrevea account for free at www.myprevea.com or download as an app. It takes just minutes to set up an account, complete a short assessment, and register for a test.
  2. If you do not have access or are unable to use the internet or MyPrevea, you may call (920) 272-3540 to schedule a COVID-19 test.
