- Cardiovascular disease prevention, screening and education
- Management of coronary artery disease, valve disorders, heart failure, rhythm disorders, lung injury and infection
- Lifestyle modification including healthy eating, weight management, exercise and tobacco cessation
- Cholesterol and blood pressure management
- Work in collaboration with Prevea’s heart and lung surgeons for pre-surgery evaluation, post-operative care, and follow-up appointments
- Coordinate post-surgery discharge planning and care
