What is hip preservation? 

Hip preservation describes a group of surgical procedures that aim to fix various causes of hip pain and dysfunction in a normal, pre-arthritic hip joint to improve an individual’s hip function and reduce hip pain - while simultaneously preserving the natural hip joint and delaying or avoiding the need for total hip replacement.  

Who is a candidate for hip preservation?

While total hip replacement is a necessary and highly effective treatment for many patients, especially those with moderate-to-severe osteoarthritis, there are some conditions – if caught early enough – in which the natural joint, cartilage and tissues can be preserved. There is significant benefit to this as our natural joints and tissues are “living”, allowing them to regenerate and last for decades, provided the proper treatment is applied -- unlike artificial hip joints in a total hip replacement, which will inevitably wear out. 

While hip preservation procedures are most often performed on young and active individuals suffering from painful hip conditions, they may also be recommended for aging adults. This may include adults with physically demanding jobs or lifestyles, or anyone with recurring hip pain or problems caused by conditions such as femoroacetabular impingement (FAI or hip impingement), hip dysplasia, labral tears, rotation abnormalities of the thigh bone (femur), and traumatic injuries. 

Anyone with recurring and/or undiagnosed pain in their hip, groin or buttocks should be evaluated for hip preservation treatment.  

What hip preservation procedures are offered at Prevea Orthopedics? 

The following hip preservation procedures are performed by Michael K. Ryan, MD, a dual board-certified and fellowship-trained orthopedic and sports medicine surgeon, at HSHS St. Mary’s Hospital Medical Center in Green Bay, Wis. To make an appointment with Dr. Ryan, please call (920) 272-3300.

Hip arthroscopy  

A minimally invasive technique to repair common and complex hip injuries such as hip impingement, labral tears, cartilage injuries, loose bodies, snapping hip and hip instability. Several small incisions are made around the hip joint, and a small camera is inserted to help the surgeon identify the area(s) of the joint in need of repair. Additional tools are used to repair the area(s) of concern and correct the cause of the damage. The minimally invasive nature of this procedure allows for a much quicker recovery time for the patient as there is less swelling, and tissue and muscle disruption. 

Periacetabular Osteotomy (PAO) 

A surgical treatment for hip dysplasia or other hip socket abnormalities in which the surgeon realigns the patient’s hip socket to improve function, minimize pain and delay the onset and progression of arthritis. Dr. Ryan completed a fellowship and was trained to perform the procedure in Bern, Switzerland, where PAO (also known as the Bernese osteotomy) was pioneered. Surgical Dislocation of the Hip (SDH)  

An open surgical procedure for more complex conditions that cause hip pain, such as FAI/hip impingement, deformities, and traumatic injuries; or for some in which arthroscopy or other surgeries have been unsuccessful. In SDH, the surgeon surgically dislocates the ball-and-socket joint so the natural hip joint can be repaired.  

Femoral Rotational Osteotomy  

A surgical procedure performed to correct specific deformities of the femur (long bone in the upper leg, also called the thigh bone) and hip joint that may be causing a variety of problems including cartilage damage, labrum tears, and various forms of hip impingement. In this procedure, the femur is realigned to resolve or prevent those problems from occurring.
