Eligibility

The Kaytee Health & Wellness Center is available to all Central Garden & Pet/Kaytee employees as well as their spouse and dependents regardless of their medical insurance plan.

Cost

  • $0 per preventive care visit 
  • $5 per family medicine visit
  • $5 per physical therapy visit
  • $5 per lab visit regardless of amount of labs (some exclusions may apply)
  • $0 for first three Prescription for Wellness sessions per calendar year; $35 for each session thereafter
  • $35 for each Prevea Virtual Care visit
  • If radiology or pathology services are required, those will be billed directly to your medical plan.

Payment will be collected at time of service with credit/debit/HSA card.

Family Medicine

Monday: 7 a.m. to 5 p.m.

Wednesday: 6 a.m. to 4:30 p.m.

Friday: 6 a.m. to 12:30 p.m.

Physical Therapy
Monday: 7 a.m. to 4 p.m.
Wednesday: 7 a.m. to 4 p.m.

Available virtually or at various Prevea Health Centers.
Monday through Friday.

  • Routine medical care for children, adults and elderly including medication management
  • Chronic disease management for high blood pressure and cholesterol, hyperlipidemia, diabetes, dyslipidemia, COPD, asthma, thyroid problems
  • Minor office procedures such as skin lesion removal/biopsy, stitches
  • Nicotine cessation
  • Basic mental health including anxiety and depression
  • Diagnosis and treatment of workplace injuries
  • Occupational health services including DOT exams

Virtual appointments available if appropriate.

Evaluation and treatment of non-work related and work-related injuries, not limited to:
  • Fractures
