Welcome to the Kimberly Area School District Health & Wellness Center

Here, you will find a passion for health care excellence to help prevent illness and keep you well. You receive the same high-quality care and services from primary care providers, physical therapists, and other specialists in this health and wellness center as you would at a typical health center. It’s all provided to you in a convenient location and at an affordable rate, thanks to KASD’s commitment to your health and wellbeing.

Eligibility

The Kimberly Area School District Health & Wellness Center is available to all employees, spouses and dependents as well as retirees covered by the Kimberly Area School District medical plan.

Cost

  • $0 per family medicine visit
  • $0 per physical therapy visit
  • $0 for each Prevea Partnered Health visit
  • $20 for each virtual care visit for those on the Kimberly Area School District medical plan
  • $35 for each virtual care visit for those not on the Kimberly Area School District medical plan
  • $0 per lab test
  • If radiology or pathology services are required, those will be billed directly to your medical plan.

Summer hours (early June through mid-August) 
Monday: 7:30 a.m. to 5 p.m.
Tuesday: 9:15 a.m. to 4:45 p.m.
Wednesday: 7 a.m. to 2:30 p.m.
Thursday: 12:45 to 4:45 p.m.

  • Routine medical care for children, adults and elderly including medication management
  • Chronic disease management for high blood pressure and cholesterol, hyperlipidemia, diabetes, dyslipidemia, COPD, asthma, thyroid problems
  • Minor office procedures such as skin lesion removal/biopsy, stitches
  • Nicotine cessation
  • Basic mental health including anxiety and depression
  • Diagnosis and treatment of workplace injuries
  • Occupational health services including DOT exams
