Eligibility

Health & Wellness Services are available to all PLENCO retirees, employees, spouses and dependents on PLENCO’s health plan. Prevea Fast Track Injury Prevention Program and physical therapy is available to all PLENCO employees.

Cost

  • $0 per preventive care visit
  • $0/$25* per primary care visit (copay/HDHP)
  • $0 per physical therapy visit (onsite at PLENCO only)
  • $0 per Prevea Fast Track Injury Prevention Program visit
  • $0 per immunization visit
  • $0 per lab visit (some exclusions may apply)
  • Lab tests sent to an external laboratory, as well as any radiology or pathology services, will be billed directly to your medical plan.

Payment will be collected at time of service with credit/debit/HSA card, cash or check.

* This benefit isn't available to people who have Medicare Part B or C, Medicaid or TRICARE.

Monday: 7 a.m. to 11 a.m.

  • Acute care such as allergies, bites and stings, burns and sunburn, coughs and colds, ear pain, flu, headache, injuries/non-surgical fracture and musculoskeletal care, laceration evaluations, pink eye/stye, sinus infections, skin infections, sore throats, UTI/bladder infection
  • Routine medical care for children, adults and elderly including medication management
  • Chronic disease management for high blood pressure and cholesterol, hyperlipidemia, diabetes, dyslipidemia, COPD, asthma, thyroid problems
  • Minor office procedures such as skin lesion removal/biopsy, stitches
  • Nicotine cessation
  • Basic mental health including anxiety and depression
  • Diagnosis and treatment of workplace injuries
  • Occupational health services including DOT exams

Virtual appointments available if appropriate.

Evaluation and treatment of non-work related and work-related injuries, not limited to:
  • Fractures
  • Sprains
  • Strains
  • Surgery recovery
  • Injury prevention management and risk assessments.
  • Musculoskeletal fatigue, discomfort or pain management (personal or work-related).
  • Ergonomic workstation assessments and suggestions offered to reduce risk for injury, such as reducing or eliminating the need for excessive reaching, bending and twisting.
  • Stretching and/or exercise programs.
  • Braces or supports guidance.
  • Rapid screen diagnostic tests such as rapid strep, urinalysis, pregnancy, mononucleosis and blood glucose
