At Prevea Kohler Health Center, you have access to a variety of services including urgent care, primary care (family medicine and internal medicine), behavioral care, chiropractic care, orthopedics and sports medicine, physical therapy, preventive care, radiology and labs.

Eligibility

The Rehlko Health & Wellness services are available to all Rehlko employees regardless of medical insurance plan. Services are also available to the employee’s spouse and dependents covered by the Rehlko medical plan.*

Cost

  • $0 per preventive care visit
  • $0 per X-ray
  • $0 per each lab test (some exclusions may apply)
  • $0 per visit if enrolled in Rehlko's COUPE plan
  • $10 per visit if enrolled in Rehlko's Anthem PPO/HDHP
  • $40 per visit if not enrolled in Rehlko's medical plan and/or those in a waiting period before enrollment
  • Immunization, pathology, injection services and supplies are billed directly to your medical plan.

Payment will be collected at time of service with credit/debit/HSA card or check.

* This benefit isn't available to people who have government health coverage, such as Medicare, Medicaid or TRICARE.

  • Routine medical care for children, adults and elderly including medication management
  • Chronic disease management for high blood pressure and cholesterol, hyperlipidemia, diabetes, dyslipidemia, COPD, asthma, thyroid problems
  • Minor office procedures such as skin lesion removal/biopsy, stitches
  • Nicotine cessation
  • Basic mental health including anxiety and depression
  • Diagnosis and treatment of workplace injuries
  • Occupational health services including DOT exams
