Our team provides personalized, patient-centered care for individuals and families, supporting healthy habits across all ages. Through the Optimal Well-Being path, we focus on sustainable lifestyle changes that support physical, emotional, social and overall well-being. 

Your care is tailored to your goals and may include lifestyle coaching, fitness and stress-management support or focused nutrition guidance. When appropriate, your specialist works closely with your primary care provider and other members of your care team to ensure coordinated, well-rounded care.

Meet Your Care Team 

Health & Wellness Specialist 
Focus: lifestyle habits, behavior change, and whole-person wellness for adults and children. 

Health & Wellness Specialists support sustainable lifestyle changes for individuals and families. Services are tailored to all ages, including children, with an emphasis on healthy routines, movement and family-centered care. 

Focus Areas: 
  • Age-appropriate fitness and movement programs, including play-based activity for children. 
  • Exercise and physical activity guidance for all ages. 
  • Family-centered lifestyle coaching to support pediatric wellbeing. 
  • Lifestyle modifications to reduce chronic disease risk, such as high cholesterol, prediabetes and high blood pressure. 
  • Nicotine cessation support (smoking and other nicotine products). 
  • Pediatric healthy growth and development support. 
  • SMART goal setting and accountability. 
  • Stress reduction and stress-management techniques. 
  • Time-management and routine planning for individuals and families. 
  • Weight loss, weight gain, and weight maintenance.


Registered Dietitian Nutritionist (RDN)
Focus: clinical nutrition and personalized eating plans, across the lifespan.

Registered Dietitian Nutritionists provide evidence-based nutrition care for children, adolescents and adults, tailoring recommendations to developmental stage, medical needs and family preferences.

Focus Areas: 

  • Family-based nutrition education and meal-planning support.
  • Individualized nutrition assessment and counseling for all ages.
  • Medical nutrition therapy and therapeutic diets, including:
    • Diabetes
    • Cardiac health
    • High cholesterol
  • Food allergies and sensitivities
  • Mindful and intuitive eating approaches.
  • Nutrition education and clarification of fad diets.
  • Pediatric nutrition guidance to support healthy growth and development. 
  • Personalized weight-management strategies.
  • SMART goal setting related to nutrition.
  • Targeted nutrition interventions based on individual health needs.


Pediatric & Family-Centered Care (Integrated Approach)
For pediatric patients, care is designed to support healthy growth, development and lifelong habits. Providers work closely with families to encourage age-appropriate nutrition, physical activity and routines that fit into everyday life. 

This family-centered approach helps: 

  • Promote healthy growth and development. 
  • Encourage positive relationships with food and movement. 
  • Support sustainable lifestyle changes for the entire family


Sessions

During your initial, free informational session, you will collaborate with a health and wellness specialist to discuss what you want to change, set measurable goals that are driven by you and establish consistent follow-up coaching for maximum results. 

Remaining sessions are $35 each. 

You may also add on additional services, such as lipid and glucose blood work for $20 and a Fit3D body scan for $50.
